Added a runtime traceability foundation that records not only final AI results, but also what happened during processing.

By keeping related execution steps connected, it is easier to investigate where and how a problem occurred when something goes wrong. This provides a clearer evidence trail for debugging and quality verification.

Because the recorded execution history can also be used by later validation and review processes, it now serves as one of the foundations for AscenderAI's broader quality-control workflow.
